Find an Echo
Unmissable. The first Echo appears during the cutscene at the vantage point in Chapter 1:

Interact with an Anomaly
Unmissable. Use the EM Scanner to clear an Anomaly:
Augmented Reality
Use the Navigator for two consecutive minutes
Use the Pathfinder (Q) or EM Scanner (E) for two consecutive minutes.
Cliffhanger
Perform 15 Perfect Grabs in a row while climbing
Perform 15 Perfect Grabs in a row. You can enable the Automatic Ledge Catch gameplay modifier to automatically trigger a Perfect Grab each time you jump toward a ledge, trivializing the achievement.
High-wire Act
Perform 3 Perfect Balances
Perform 3 Perfect Balances while crossing narrow bridges (don’t lean too far to one side):
Call of the Void
Die 20 times from a fall
Die 20 times by falling. Make sure the Game Over screen message reads Ariane fell to her death.
Killed by the Nemesis
Killed by the Nemesis 10 times
Die 10 times to the Nemesis, the alien creature introduced in Chapter 4:
Whisperstep
Escape the Nemesis Arena without being detected
Complete any Nemesis arena without being detected. Follow this video for an easy strategy in Chapter 4:
If you get spotted during this encounter, simply reload the last checkpoint and try again.
Echo Seeker
Find the hidden Echo in “The Onyx Forest” Chapter
In Chapter 4, enter the cave in the melting area:
Distraction Master
Lure the Nemesis away with Decoy Anomalies 10 times
Use the EM Scanner to activate 10 Decoy Anomalies, which are introduced in Chapter 5:
Nice Try
Fail the Commander’s door code 3 times in a row
In Chapter 6, enter a wrong password on the Commander’s door 3 times in a row:
Absolute Zero
Freeze to death 3 times in “The Storm” Chapter
In Chapter 7, die 3 times to the equatorial storm:
Orpheus
Cross the river without dying in the “The Styx” Chapter
In Chapter 10, complete the final section without dying:
If you die, load a previous save from the Extras – Rollback Save menu.

Has anybody found the last marker yet. I came across the Styx River one but currently sitting at 96% so I guess I’m missing one and redid all chapters. Kinda losing my mind 😆 I’m so ready to be done with this game. They should of added to where you can see how many secondary objectives you got on each chapter in chapter select like everything else.
I completely agree about tracking in each chapter.
I think the last marker is bugged and simply doesn’t appear in a specific location. It can’t be that so many people can’t find the last marker.
Yeah for sure. I’m probably just going to give up for now. This has given me a headache 🤣 was talking with Maka hopefully he figures it out soon or someone does. I can’t bring myself to go thru anymore chapters rechecking these again.
Did you get the storm one at the start of chapter 9?
Also someone else below mention a missing one in chapter 4 when you encounter the nemesis, been through it again and not found where they mean. As there’s 3 encounters
@EvanE shared the missing one, it’s the “A Way Out?” marker in Chapter 7.
I’m missing one secondary marker, it was two but I scanned the equatorial storm at the start of chapter 9 and it updated my percentage. Anyone got any idea which I’m missing? As I’ve gone through each chapter 3-4 times now. TIA
Do you know if The Source counted for you? And do you remember the secondary marker that’s missing in the guide?
Is the source supposed to be a marker too in one of the chapters? As I don’t remember it, and the only one I remember is the one I mentioned in my previous comment, after reading somebody else comment it to you. I wasn’t really following a guide up until chapter 8 or so, so I can’t remember which ones you haven’t listed that I got.
The marker for The Source is white, so I suppose it should count as a secondary marker. It first appeared in Chapter 7, yet was never counted, not even in the final chapter.
I think the one Ingolf mentioned might be the missing one, but I haven’t been able to find a marker in either of the 3 encounters with the nemesis in chapter 4.
I found one marker location in the Stige level, by the level name the Stige River itself
Do you mean The Source, or was it another marker?
After the second fence, which you tear off with a hook, you approach a ledge and there is a wide river. You point somewhere in the center and the marker shows the name of the river.

@Ingolf can you explain where in chapter 4 you mean, as there are three times you have to sneak past the nemesis. I’ve checked all three and can’t find a missing secondary marker. What exactly is it marking ?
I think @Ingolf meant the Whisperstep achievement, it was missing earlier.

Chapter 7 in the the cave where you have to get past the nemesis, if you scan towards the area you have to go which is a pool of water you have to slide down you will get a secondary marker called “A Way Out?”.
